A duplicate card can be requested from the CSCS Helpline on 0844 576 8777, this will cost £30.00 and payment can be taken over phone using a credit/debit card if you are happy to use the existing photo. If you want to update your photo then a pre-populated form can be sent to you to attach a new photo and the appropriate payment.
